AZA Rodent Insectivore and Lagomorph TAG

 

The information below is copied directly from the regulation 
(which is found at the end of the Federal Register Notice) that specifies the information 
we need in a permit request.  Each question in paragraph B must be answered.

Any person or group wishing to move prairie dogs 
(wild or captive) from one place to another must make a written request to FDA to ask for permission to move those animals.  

  

    (A) To obtain such written permission from FDA, you must send a written request to the Division of Compliance 
         (HFV-230), Center for Veterinary Medicine, Food and Drug Administration, 
          7500 Standish Pl., Rockville, MD 20855, Attn: Listed Animal Permit Official. 
          You may also fax your request to the Division of Compliance 
          (using the same address in the previous sentence) at 301-827-1498.

 

    (B) Your request must state the reasons why you need an exemption, _________________________________,

 describe the animals involved, ____________________________________________________________________, 

 describe the number of animals involved, ____________________________________________________________,

 describe how the animals will be transported (including carrying containers or cages, precautions for handlers, 
 types of vehicles used, and other procedures to minimize exposure of animals and precautions to prevent animals 
 from escaping into the environment),________________________________________________________________,

 describe any holding facilities, ____________________________________________________________________, 

 quarantine procedures, __________________________________________________________________________, 

 and/or veterinarian evaluation involved in the animals' movement, ________________________________________,

 and explain why an exemption will not result in the spread of monkeypox within the United States, 
______________________________________________________________________________________________.

 

Additionally, we need to know the physical address where the animals are currently located and the physical address where the animals are to be transported, _____________________________________________________________________________________________, 

as well as the names and addresses (if different) of the people responsible for the animals involved,______________________________________________________________________________________.
